A few years ago, our Founder's Mother was brought to an E/R, unconscious. She finally regained consciousness, but the Doctors could not get in touch with any family members to ASK if there was any Medical Directive in Place.
Fast forward to NOW, and we've put together a data center that handles the storage and retrieval of Medical Directives and Legal Directives. Log in to the WebSite, use our Phone App, then any Doctor or Nurse can retrieve your Medical Directive into their email or fax exactly when they need it.
Being Unconscious at a Medical Facility is a scary thing for everyone - having a Medical Directive in place removes a level of doubt and worry.
How it works

Fill out your forms
Most of the forms are PDF-fillable, you fill it out on your computer then print it out. You Sign, a Notary Signs, A Witness Signs
Fax/Email your forms to us.
We take signed forms directly via fax. Any email attachments must be PDF, so please scan your signed into multi-page PDF format prior.
Hospital retrieves your form
You receive a Medical Directives Card via email - Print Out and keep in your wallet or purse. The hospital will read the retrieval directions and your signed or notarized forms arrive via fax or email to the Hospital.
